I'm 5'3" and 110 lbs. I wear a size 34D bra. I bought the Small/Medium and the top portion is way too large for my breasts even with the adjustable straps cranked as tight as they will go. I feel like Sandra Bullock in The Proposal where Betty White says "well they're in there somewhere". The bottom half fits nicely though.
